Mrs. Wilkerson is a 55-year-old female
with the history of borderline diabetes, hypertension, asthma, rheumatoid arthritis,
obesity, and chronic obstructive pulmonary disease. Mrs. Wilkerson has been receiving
medical treatment at Forum Pain Management Clinic for
over five years. After conservative treatments had failed to resolve her pain, more
aggressive pain management was initiated. Mrs. Wilkerson has had an implantable morphine
pump for over four years and a spinal cord stimulator for over two years.

Mr. Otto is an 87-year-old gentleman diagnosed with spinal stenosis. As a
result of his stenosis, he had constant pain in his lower back, hip, and leg. He received
a series of three epidural injections, plus pain medications and physical therapy. His
pain is now under control.

Mrs. Saulich is a 48-year-old female who
suffers from chronic knee pain due to arthritis. She has undergone two arthroscopic
surgeries on her left knee, one in 1989 and the other one in 1997. She is not a candidate
for total knee arthroplasty due to her age. She did not respond to medications, physical
therapy, or steroid injections. A Hyalgan injection was therefore recommended. She reports

become more ambulatory, which has significantly improved her quality of life. She does
require small doses of narcotic and other medications for additional pain relief.

has been attending Forum Pain Management Clinic for
over three years and has an implantable morphine pump for her pain. Her headaches are
completely resolved and her pain is significantly reduced.
